Most run a soft permadeath system — a character KIA in a major operation may permadie, but routine training accidents or minor combat injuries are recoverable. The KIA decision usually requires command-staff and player consent.

Some do. Joint operations between military and law enforcement (counter-narcotics, anti-terrorism) create some of the best cross-faction RP available. Servers with strong both-sides usually have formal agreements about jurisdiction and operational coordination.

Most are. The mature themes (combat, military discipline, complex political scenarios) lend themselves to adult communities. Some 16+ military servers exist but are less common.
